Documents Required for a Reverse Mortgage

Please provide the following items at the time of application:

• Copy of Social Security Card
• Proof of Age – copy of driver's license or birth certificate
• Copy of recent property tax bill
• Copy of the declarations page of the homeowner's insurance policy or a bill showing that the policy is current
• Copy of death certificate for any deceased title holders/owners of the property
• Copy of Power of Attorney (if applicable)
• Counseling Certificate from a HUD approved agency as proof of attendance to an information seminar on reverse mortgages
